    How long does Naltrexone stays in your system?

    Hi All,

    It s been along time.

    How much does naltrexone stays in your system?

    For example if I am taking today 25mg and tomorrow 25 mg, will it stay for at least 12 h. Or do you feel the effect after 24 h?
    Or it depends on how long do you take it.

    If you take 25mg it will stay in your system for 12 hours and if you take 50mg it will stay in your system for 24 hours, although it will still be washing out of your system the next day after the 24 hours. that is why you are recommended, if you are exercising, to wait another day before commencing any exercising.

      it does not matter how long you are taking it, it is still going straight into your system. your are recommended to take 25mg for the first two times of taking it due to any side effects i.e. nausea.

        I'm not sure about that part I've underlined Mousse, I thought in theory 25mg covers your endorphin receptors 100% for up to twelve hours, but it will stay in your system for much longer than that. If that's correct then, say you need painkillers for surgery, you would need a good few wash-out days after having 50mg, unless the medical staff were alerted and gave you non-opioid alternatives.

          Yes I thought the 25mg will cover your endorphin receptors 100% for 12 hours the same as 50mg for 24 hours but that both take another day to wash out of your system? If Im wrong I stand corrected! Please let us know 8, I don;t have the book with me here..

            Ah, I wouldn't like to offer any medical advice Mousse, hopefully someone with a good understanding will offer their thoughts, but I personally think you have the right kind of idea there. I guess it will depend on the physiology and metabolism of each person, too.

              How long does Naltrexone stays in your system?

              If you look up information about naltrexone you'll see a reference to something called it's "half life" which is the amount of time it takes it to decrease to half it's original strength. Wikipedia says naltrexone's half life is 4 hours but I've seen other sources give a higher number. I think if we want to figure out how long naltrexone blocks the opioid-receptors for we need to know how much naltrexone the body needs to get that effect (if for instance that number was say 12.5 mg, 50 mg would be effective for 8 hours), and I don't know that. What I can say though is that (if my understanding is correct) 50 mg decays to 25 mg in 4 hours, so a 50 mg pill will give you 4 hours more of the effect than a 25 mg pill will.

                I don't think we can trust Wikipedia on such an important topic. Dr Eskapa I believe says that 50mg should block up to 100% of the opioid receptors for up to 24 hours, which seems to totally contradict that Wikipedia info. That would mean days
                before it would be totally out of your system, (and thus days before any opioid-based pain relief medication would work as it should if, gods forbid, one was in an emergency room setting, for example after an accident). Perhaps someone should email Dr Eskapa to clear this up?

                  I don't know the answer but I write the date I last took one on the medic-alert card in my wallet in hopes that if necessary, medicos will be able to sort it out.

                    There is no contradiction. I chose the number 12.5 to illustrate my point because that's the number after 2 4 hour periods (so it's easier to understand). Here's naltrexone halving over 24 hours (after the initial 30-60 mins it takes to enter the system):

                    0 hours: 50 mg
                    4 hours: 25 mg
                    8 hours: 12.5 mg
                    12 hours: 6.25 mg
                    16 hours: 3.125 mg
                    20 hours: 1.5625 mg
                    24 hours: 0.78125 mg

                    So, if the amount of naltrexone that one needs to get the effect is 0.5 mg, we can see that a 50 mg pill would still work after 24 hours. I was trying to explain the mechanism - I do not know the amount in mgs that one needs in one's system for naltrexone to block all the opioid-receptors.

                      GettingBetter, with all due respect that seems to be all speculation on your part based on a figure given by Wikipedia, which I don't think is at all helpful. If it did only take 0.5mg (just your example) to block receptors then we would be told to take far less than one 50mg tablet if we are drinking for very much less than 24 hours, but we are not. There's the contradiction.

                        Well, it's not just Wikipedia (which by the way is usually an excellent source - it's been found to be more reliable than Encyclopedia Brittanica on average).

                        Here's another source which says it's 4 hours, and here's one more.

                        No, there's no contradiction. If 0.5 mg was the amount necessary to block all the opioid-receptors, a 1 mg pill would only last 4 hours before dropping under that number. 50 mg leaves 0.78125 mg after 24 hours. Look at the numbers in my last post.

                          Thank you for the sources GettingBetter, but still I would rather this is something cleared up by either Dr Eskapa or Dr Sinclair, or another doctor with expert knowledge of Naltrexone use.

                          And I'm sure that Wikipedia can be correct, I'm just personally not willing to trust it alone on what is potentially a life-threatening (if you have an accident and are taken to ER) scenario, and they give you opioid-based medications.

                          The contradiction is that all of the studies, all of Dr Sinclair's research and practical experience, and all of Dr Eskapa's book is based on taking 50mg each and every time you drink. If your speculation's true (that you need only a fraction of the dose if you're drinking in a far shorter timeframe than 24 hours) then do you not think they would have touched on that at some point in the years they've been working with The Sinclair Method?

                            No, because there is no contradiction. If I am going on a 24 hour bender (and I have done, many times) it is important that the drug continues to work during that time. A 50 mg naltrexone pill with a 4 hour half life will leave 0.78125 mg after 24 hours. If Dr Sinclair and/or Dr Eskapa say that the opiod-receptor blocking affects of 50 mg of Naltrexone last 24 hours I have no reason to disbelieve them. Mathematically that means the amount the body needs to be effective is less than 0.78125 mg. That's just how the maths works out. You can see that right?

                            I am not encouraging people to play with the dosage, I am merely trying to explain what a "half life" and the nature of exponential decay are.
